County Sheriff's of Colorado Address Change / Permit Lost or Destroyed Form

All applicants will need a $100.00 processing fee in the form of a cashier's check, money order (no cash please), made payable to the Moffat County Sheriff's Office (MCSO).

In addition NEW PERMITS a $52.50 processing fee in the form of a cashier’s check or money order, made payable to the Moffat County Sheriff's Office. This fee pays for a state and national criminal history check (fingerprints) and for an Insta-check (NICS) per Statute. This is required for all regular civilian applicants. For RENEWAL PERMITS a $13.00 MONEY ORDER made payable to the Moffat County Sheriff's Office for the CBI fee is required, as well as a $50.00 MONEY ORDER made payable to the Moffat County Sheriff's Office for the sheriff's fee.

To conduct a thorough background investigation and comply with State Laws, it is required that all applicants be fingerprinted and photographed by the Sheriff's Office. The Moffat County Detention Center will take photographs and fingerprints everyday.

Your application can not be processed until your photograph and fingerprints are taken..
